What is Resource Navigation?
Resource navigation is the process of helping individuals and families navigate the complexities of the health and human services delivery system by assisting with identifying and accessing the services available to help them thrive. Our experienced team works closely with you to understand your unique situation and connect you to local, state, and federal programs designed to improve your quality of life.
Our Resource Navigation Services are designed to:
• Provide personalized assistance in accessing resources tailored to your specific needs.
• Offer guidance through complex processes, such as applying for benefits or scheduling health appointments.
• Ensure no one falls through the cracks by advocating for individuals and families to receive the support they deserve.
Services We Offer
Our team connects you to a wide range of essential services, including:
• Housing Assistance
o Help with finding temporary or permanent housing solutions
o Connection to homeless shelters, housing vouchers, and eviction prevention programs
• Financial Assistance & Benefits
o Support with applications for food assistance (SNAP), utility assistance, and more
• Behavioral Health Services
o Referrals to mental health providers and counseling services
o Assistance with connecting to crisis intervention services or substance abuse support
• Transportation Services
o Help navigating local transportation options
o Ride United Lyft partnership
